I assume faxgetty is running?  If so, set ServerTracing to 0xFFF in 
your modem config file and take a look at what's happening in the 
syslog.

Prior to 4.2.0 HylaFAX initialized modems twice before every faxsend.  
FaxQueuer would first initialize it, and then faxsend would 
re-initialize it.  In 4.2.0 we just simply prevent faxsend from 
re-initializing the modem.  As everyone *should* be using faxq to 
dispatch faxsend this should be no problem.
