The command-response interaction here between HylaFAX and the modem is interesting. Notice that the AT+FTH=3 command response ends up being NO CARRIER instead of OK or ERROR. The response is outside of T.31 spec, I think. However, that indication is used by other digital modems that a disconnection occurred at that moment. In fact, the ERROR response to AT+FRM=146 seems to confirm this conclusion.


So what it looks like from the log is that the caller sent TSI, DCS, and TCF... and then promptly hung up. Now, that may not be what happened, but that's what the logs tell us. The disconnection could, of course, be happening for other reasons, but the logs aren't going to tell us what. Also, there could be a problem with the modem driver/firmware, and you'd have to consult with the manufacturer if that is the case.

I would suggest first checking with the caller to see if they know what happened. If they intentionally disconnected (turned off the fax machine or pressed "stop"), then that's pretty clear. But if they also are seeing an error, then it may have to do with phone lines, a PBX, or perhaps the modem itself that is causing the disconnection.
