I use the -F flag to set the tagline, like this:

-F "Me 1.555.555.1212|%%l|Page %%P of %%T"

(Substitute something else for 'Me' and the number, of course).

Check out man hylafax-config and look at the TagLineFormat parameter for
information on how to constuct the string following -F.
