Our customers occasionally report this problem ... once you've figured it out once it's easy to recognize. The sender's sheet feeder seems to have goofed and fed the first two sheets as if they were one. So what you have is one very long page containing pages 1 & 2 (scaled to fit), and then a short one containing page 3. HylaFAX sometimes scales all pages according to the longest page, which may be why the third page is also affected.
