Are you using PAM to authenticate the users or hosts.hfaxd?
A dodgey PAM module will cause the hylafax process to die. I've managed
it myself quite a few times :)
If you're not sure look in /etc/pam.d for a file called hylafax.

I've been running 4.1.8 on this machine with no problem.
