[hylafax-users] format_failed when sending PDF

Up until today I was able to send using PDF, now I receive the following 
error. In the log I get CONVERT DOCUMENT: exit status 0x100, then notify 
throws "format_failed".

Your facsimile job to xxx-xxx-xxxx was not sent because document conversion
to facsimile failed.  The output from the converter program was:
