this could be a lot of problems. Please check 
http://www.hylafax.org/troubleshooting.html and
http://www.hylafax.org/HylaFAQ/index.html
and then give some more infos about your system
(Modem, Version of HylaFax, Version of Linux,
some logs etc.)
