There was a bug in 4.2.1 that required this patch:

This bug caused uuencode to not be able to use base64 encoding. Some
MUAs such as Outlook may not like x-uuencode encoding.
The easy way to fix this is to change ENCODING in
/var/spool/hylafax/etc/setup.cache to say "ENCODING=base64".
