This is part of your problem, at least. All those faxq processes are 
fighting with one another. There should generally be only one long-lived 
faxq process in the process table.

Stop HylaFAX (/etc/init.d/hylafax stop), kill any remaining faxq processes, 
then restart HylaFAX.
