Oct 19 16:36:28.02: [19275]: --> [0:]
Here's the error. See that the modem did not respond with a CONNECT
message after AT+FTH=3 ? That indicates a problem of some kind, and,
as we can see below, it occurred because the modem was in the middle of
a reset.
Oct 19 16:36:28.02: [19275]: RECV send PPR (partial page request)
Oct 19 16:36:28.02: [19275]: <-- [10:AT+FRM=72\r]
Oct 19 16:36:28.27: [19275]: --> [9:AT+FRM=72]
Notice here that we have echo turned on. However, HylaFAX disables
echo before every fax, so we know that the modem was reset.
Oct 19 16:36:28.27: [19275]: RECV keeping unconfirmed page
Oct 19 16:36:28.30: [19275]: RECV: 1792 bytes of data, 16 total lines

Oct 19 16:36:28.36: [19275]: RECV FAX (000001024):
recvq/fax000000293.tif from 81454765271, route to <unspecified>, 4

Oct 19 16:36:28.38: [19275]: RECV FAX: Failed to properly detect
high-speed data carrier.
Oct 19 16:36:28.38: [19275]: <-- [9:AT+FTH=3\r]
Oct 19 16:36:28.38: [19275]: --> [5:ERROR]
And the error message here tells us that we're on-hook, which confirms
that the modem did reset.
My guess is that there is a bug in the USR firmware associated with
AT+FRS commands. You could try disabling +FRS and +FTS. Look at the
Topic config file (config/topic) on how to do this.
