I think that the second patch on:

http://bugs.hylafax.org/bugzilla/show_bug.cgi?id=454

addressed this issue.  There was a typo (mixup between 
CIDNameAnswerLength and CIDNumberAnswerLength) which basically disabled 
CIDNameAnswerLength.  It's fixed in 4.2.0.  Upgrade if you can, please, 
and then if the problem persists let us know.
