You only need one modem per line.  One line can support as many dialed 
numbers as the telco will allow.

You'll need to talk about this with your telco.  However, if you're 
talking about more than two incoming fax lines and two associated DID 
lines, then you'll probably be better off getting digital lines and 
digital equipment.
