You're probably missing "pagesizes" and "typerules", I think you will 
have to put them in /usr/local/lib/fax. You can find the files in the 
util directory (source) or check your server installation, I had the 
same problem a few months ago when I compiled 4.1.5 from source.
You can set your host parameters in hyla.conf.
