In HylaFAX, the line on the top of the page is called the Tag Line
and it can be modified by changing the TagLineFormat option in the
SPOOL/etc/config.[device] file.

Since the Tag Line can be customized in many ways, I would suggest
that you consulted the hylfax-config manual page and the TagLine
section in setup-advanced.html of the HylaFAX documentation.
