We've been chasing a similar issue for a few months now, and think we may
have it fixed. I suspect you're encountering the same problem we have been
seeing on some of our high-end fax servers (useful for faxing over one or
more T1s). Our servers are dual-CPU machines with high-performance
filesystems, ... it seems heavy client/server I/O and a powerful server
combine to trigger this issue.
