I remember seeing this behavior from USR PCI modems before (two CONNECT 
messages before sending TCF), but I don't know if it was ever 
resolved.  It's certainly a firmware issue.

Try setting Class1TCFWaitCmd (see 'man hylafax-config') to AT+FTS=7.
