FreeBSD style ;-) (sorry I know not about Linux). Here is how I made my
coverpage, step-by-step.
On a Windows PC, using MS Word...
1. Install the Apple laserwriter Printer driver (set it to print to file)
2. Open MS Word.
3. Insert your company logo at the place appropriate for you (I did mine
   with strict TPC.INT policy, yours may be different on the placement).
   I'm assumming you've got the logo as .GIF of JPEG
4. Print the document to a file - name the file faxcover.ps (whatever it
   is called in Linux. 
5. Ftp this file to your *NIX box and you may use dos2unix to convert it
   so that those ugly ^M are removed.
6. Call the resulting file faxcover.ps (is that what it is called in
   linux?). Check permissions just to make sure.

..and you should have your cover page going out with your comapny logo...
