Unable to receive faxes

Hello everyone. I'm sure I'm going to bore you all with this age-old
question, however I have not been able to find anything in the archives that
has helped my situation.
I am running Hylafax 4.0pl2 on Mandrake Linux 6.5. My test modem is a PPI
PM144MT II.
I have set up the config files for Class 2, and I can send faxes no problem
with WHFC windows client. When I attempt to send a fax to myself from
another machine in the office, if errors with an Unknown Phase D error.
Again, I have read thru the archives and have tried several things I've
found there with no success. I'm sure that there is something that I do not
have set up correctly, but I cannot find a list of fax commands for this
modem so I am having a hard time determining what the problem is. In fact I
cannot find a listing of fax commands for ANY modem. Very frustrating. The
following are some logs of the incoming calls. I have removed the phone
numbers for security. Any help will be appreciated!

Thanks!!
