There are a couple of things you might want to look for:

Check the status of the undelivered jobs with mailq to see why exactly
they are pending.

faxmail commonly resides in /usr/local/bin/faxmail, and not in
/usr/bin/faxmail. Check against your system.

faxmail expects a destination of "-d user@host", in sendmail terms that
is "-d $u@h", not "-d $u $h". Your fax.m4 appears to be recent, so it's
somewhat strange that the '@' is missing.
