Ok, this isn't pretty, but will work. You can generate a cover sheet with
any word processor that can write Postscript to a file. I used Wordperfect.
Include in the cover sheet unique text for the
Date
To
To-Company
to-fax-number
From
Regarding
Comments
Page Count
Then, edit the Postscript file and substitute the variables the faxcover
program defines. These variables are:
/todays-date
/to
/to-company
/to-fax-number
/from
/regarding
/comments
/page-count
For testing, I put these lines at the top of the file after the BeginProlog:

/todays-date (xx/xx/xx) def
/to (Target Name) def
/to-company (Target Company) def
/to-fax-number (Target Fax) def
/from (Glenn Burkhardt) def
/regarding (the Subject) def

You can then use Ghostscript to display the cover sheet. When you're
satisfied, put %% in front of each line to comment it out.
I'm attaching copies of the original wordperfect document I created, and the
resulting cover sheet I use for you.
You might also want to pick up the book "PostScript: Language Tutorial and
Cookbook", by Adobe Systems on Addison-Wesley press.
