flexfax: Unspecified Transmit Phase B error

Hi there,

I am using HylaFAX v4.0pl1 under IRIX 6.4, binary IRIX 6.2 distribution,
with a US Robotics Sportster Faxmodem 56k with x2, FLASH 4.1.55, DSP 10.3.55

As far as I can tell everything is installed correctly, and both the 
client and server programs (running on the same SGI Octane) work fine.

When a sendfax request is submitted, the scheduler processes it, the modem
is dialed and I get the following:

Unspecified Transmit Phase B error

Obviously, something is going wrong when the modem is negotiating with the
receiving fax machine.

I have tried software and hardware control, installing binaries and compiling from 
source and everything I could find in the FAQ's and installation directions.

Can you guys offer any clues at to what may be going on?  Any help would be greatly appreciated.
