Are you shure your installation is correct. I guess you use ghostscript to 
render your coverpage into tiff. Perhaps this goes wrong. You can check this by 
creating a coverpage by hand and then take a look at the result by ghostview.

To create a coverpage you simply say 
 
faxcover -C <your faxcover template file> -f "any text" -n 12345 >/tmp/cover.ps
  
Then you can take a look at it saying ghostview /tmp/cover.ps
