Hi Mark.
If you continue to have problems then you should consider an upgrade to
4.2.5 and then possibly replacing your USR modem. HylaFAX 4.2.5 has a
number of Class 1 enhancements which may help you and many of the USR modems
have faulty fax firmware.

Changing the modem to Class 1 helped with this old fax machine. The customer
was able to fax right away. I'm monitoring the success of other faxes and am
a little nervous. I'm getting errors: "ECM page received contained no image
data." and "Failed to properly detect high-speed data carrier" from a single
source. I'll monitor for the rest of the day so that I have more specific
information.
One should always listen to Lee, especially if one is as naive about faxing
as I.

Have you tried this in Class 1?
Re-run faxaddmodem and choose Class 1.
