You didn't specify what kind of modems these are, but if they're analog voice+fax+data modems, and if your PBX supports sending DTMF to the modems after they pick up, then you can transfer the DID to the modem via that DTMF, and HylaFAX will get it that way.

Search for SHIELDED_DTMF in the archives or 'man hylafax-config'.
