Upgrading to 4.2.2 has helped a lot.

Good. A lot of work went into it. On my part, I started being very finicky about every single fax session on every single server I administer, and I got every log of every failure, and then I did what I could to address the situation. At first that was a lot of session logs every day in my inbox. (I administer a lot of fax servers, and some of them are quite busy.) These days 99.9% of the receive errors that I see are either sender-abortions or something that looks like a freak line disconnection (did the sender turn off the fax machine?).

Sep 15 11:59:43.08: [ 8292]: RECV recv DCN

This is actually a to-be-expected error. Getting DCN after sending RTN is not necessarily an "error" per-se. It just means that the sender is not going to retransmit the page. It may be technically impossible for the sending device to do that.

RTN signal from us means that we don't accept the page quality and want the sender to retransmit. Many senders will not do that, and will simply move on to the next page after retraining. In this case, the sender apparently thought that it was better to hang up and maybe try again later.
