this looks like capi4hylafax.

Did you limit the number of retries?
What value is in the "returned" line in doneq/q4 ?

What ISDN card do you use?
Did you compile hylafax or capi4hylafax yourself or do you use binary  
packages? Which version?

I think version 01.02.02 and 01.02.03 of capi4hylafax do not work with  
hylafax 4.2.x without patch or wrapper script. (fixed in the Debian  
version, see http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=293808 )

You could try the new version of capi4hylafax
