Does it need to be a Fax Cover Sheet for Hylafax? Or just a Fax cover sheet
like what you would use on a typical fax to be sent? If it is the latter,
Open up Microsoft Word (Or Oppen Office will have an equivalent) and go to
File --> New Document. There will be an option there for a fax coverpage and
it should show you all the typical fields that go onto a Fax Cover Sheet.
