WinprintHylaFAX makes the explicit assumption that something upstream is
creating the cover page, like a Word macro or Open Office template.

The FAX Wizard keeps changing dramatically from version to version of
Windows, and requires something to hook into the FAX Service API instead
of the print monitor API.  It seems awfully useful to do so, actually, I
just haven't looked into it in more depth.  I'm not sure how complex it
would have to be.
