I just updated CVS-HEAD yesterday with a working prototype config.  
Basically it boils down to:

  ModemType:  Class1
  ModemFlowControl:  rtscts

Although the card can report all of these to HylaFAX, HylaFAX currently 
can only make use of two of them.  See:

So for the time being you have to pick and choose which of DID, 
Caller*ID Number, and Caller*ID Name you wish to plug into the 
"CIDNumber" and "CIDName" slots.
