Well, first of all you would help yourself a little bit by upgrading to
4.2.1. The added ECM support will give a little bit more reliable
results. However, the real problem here is your modems. You can
expect a fair amount of problems with USR modems. The logs you
provided were typical of the kind of stuff that USRs do.
Replacement modem recommendations would be to get MultiTech
MT5634ZBA-V92 or MT5634ZPX-PCI-V92 modems.
Error Log and Modem Config (both configs are essentially identical)

Curious, the 1st line of the session log has our area-code twice,
THEN the phone#. Mean anything?
You probably have a misconfiguration somewhere in your config files.
