I have done this and found both the overlay and the blank form to each be
one page.  When viewed separately via gs, they display as a single page
each, and when combined gs also displays them as a single page, yet the fax
coverpage Hylafax generates is, for single page faxes, one page off.  One
thing that I found that is not as I originally posted is: if I generate a
two page fax with overlays on each page, it will say that 4 pages follow, a
three page fax will say 6 follow.  So, for some reason, Hylafax is seeing
these documents as two pages each when they are really only one.
