I use postfix, not sendmail, but the principals should be the same.  

I suggest using postfix if your not already locked into sendmail for
some other reason.

It's fairly simple, define a fax transport agent, which calls faxmail.
The transport agent tells the MTA what to do with the message ( or how
to send it on).
The arguments are
/usr/bin/faxmail -d -n ${user}
I believe the content of the message is piped to the above command.

Then have any mail going to @fax.mydomain.com use the fax transport
agent.  

To get notifications sent properly I also created a mapping for
@fax.mydomain.com to @mydomain.com so the notifications can get to the
proper user.  (notifications were being sent to user@xxxxxxxxxxxxxxxx
which doesn't exist)

If you're going to use postfix I can be more specific if you want.

Do you want to have the name automatically inserted into a cover sheet?

I guess it depends on what you need the mail to fax for, but I don't use
a cover sheet at all, simply put the intended recipient's name at the
top of my email and send it the fax number, just like I would when
sending a normal email

I'd also make sure  the .fax pseudodomain is properly configured  in 
"sendmail.cf".  It is a line:
"CPFAX"
