I presume you've set the parameters for "CIDNumber" and "CIDName" in 
config.dev ?  If not see man hylafax-config.  Otherwise, it may be that the 
modem can't handle caller id or it is not enabled.  You can check it with 
some modem communications program(i.e. minicom) and enter "AT+VCID=?"
It may be necessary to enable it with either "ModemResetCmds" or permanently 
enable in the modem's memory.
