Thanks for the suggestion. 
After running the command with the -vv flag, I can see that there's no
reference at all to the faxcover program. Apparently faxcover is not
being invoked at all. Could that be a bug in my sendfax version!?

I am running on hylafax 4.1.1 on Debian stable.
