3. log file says recv PIP (procedural interrupt positive)
4. extended status is "Procedure interrupt (operator intervention)"
5. short status is F

Is this right? It should be F even if it says "PIP positive"?

It should have succeeded. PIP should only cause a failure when there are more pages to send. See:

I've hung a patch there now. Can you test it?
