Not sure if this i a typo in your email, but your inittab entry should look
like
t0:345:respawn:/usr/lib/fax/faxgetty /dev/ttyS0.
                     ^^^
Also make sure that /usr/lib/fax/faxgetty is the correct location of
faxgetty for you.
