4.0pl2 is way crusty, so I may be misinterpreting the log, but it looks
like a +FRH=3 loop which was solved in 4.1-release.

I would recommend that you upgrade.  Just watch the faxcover changes and
the SPOOL_DIR changes, and you'll be fine.
