I can't help with notify.awk, but I can help on faxrcvd.  I guess that I'm
taking responsibility these days for the faxrcvd-mail scripts that are
found on the HOWTO.

If you run faxrcvd by hand, what is the output?

What are the locations of the following executables?: perl, faxinfo,
mimencode, sendmail.

One of the things to watch out for is that faxinfo is referred to in these
faxrcvd scripts as /usr/sbin/faxinfo, and a default source installation of
HylaFAX will put this in /usr/local/sbin/faxinfo instead.  If we can figure
out your problem, then I'll amend or add-to the HOWTO documentation on
this.  Also, I've met up with a number of people who didn't have mimencode
on their system.
