To make a custom coverpage, design it in something like tgif (Linux) or MS
Word (Windows).  Any of the variable information, such as "To Whom:",
should be marked such that it will be easy to find, i.e. "XXXX-To".  Then
you need to export it into EPS.  Doing that in tgif is fairly
straight-forward, but in Windows, you need to set up the Apple Laserwriter
printer to print to a file, and then you need to change the properties of
that driver to print in editable, plain-jane EPS format.  Then you print
the document to a file.  Then you go through your created EPS file and
replace the appropriate marked (XXXX) lines with the correct variable name
without the parenthesis, i.e. "(XXXX-To)" becomes "to".  See 'man faxcover'
for details on this.

The reason that you're having trouble with the make_faxcover utility found
in the HOW-TO is because it was created based on an older version of
faxcover.  Changes were made, and the faxcover documentation never changed
because of anticipated reworkings, but these reworkings just barely got done.

So, if you'd like faxcover to work as documented, and such that
make_faxcover works, then grab CVS (you've done that) and patch it with the
last patch posted at:
