Certainly.  Repost your inquiry, including the versions of HylaFAX and
your operating system, as well as the make, model, and firmware rev of
your modem, and a complete session log.  ;-)

There's been some traffic about Phase D errors somewhere recently, but
I don't remember what it said.  Was this a working installation where
something just broke, a working installation where you changed
something and now it doesn't work, or a new install?
