I find that Netscape produces nice, clean PostScript.

When I did my cover sheet, I did it in Netscape, with fake values for
the fields, and used vi to put the PostScript voodoo in (copied from
the supplied cover sheet)

I'm sure there is probably a better way then editing the PS, but it
wasn't that hard - it was the first PostScript file I had ever looked
at.
