Getting Transmit Phase B error on 1 number out of 200




My HylaFAX system is successfully sending 200 faxes a day, but one
destination always fails with the dreaded "Unspecified Transmit Phase B
error".  Going through the archives, it was said that when one encounters
such an error, one should post the error log here for others to see.  So
that's what I'm doing.  Can anyone give me a clue as to what's failing and
how I can fix it?

The fax server is running on SCO Unix OpenServer 5.0.4, HylaFAX v4.0pl2
binary.
Modems are MultiTech MT2834BR (rack-mount) recently upgraded to the latest
firmware revision.  I'm running them Class2, rtscts with the mt-1432 config
script, and using faxgetty.
