flexfax: strange random failures

Hi,

We have been running v4.0pl2 on Solaris 2.5.1/Sparc IPX for some time,
mostly inbound but with reasonably significant outbound traffic as well.

Right from the beginning, the guys operating this have reported occasional
failures to send to one particular number.  It happens when faxes are
mis-directed to this office and they resend the tiff file to the fax machine
in a another office.  Mostly this works OK, but the intriguing thing is that
when it fails (typical failure message appended), I can get it working
again for them immediately by simply removing ~fax/info/<fax number>

Anyone got any ideas?
