Hi,
I think you need to start hfaxd first.
My startup, for outgoing only, on specified modem :

/usr/local/sbin/hfaxd -i 4559 -o 4557 -s 444
sleep 3
/usr/local/sbin/faxq
sleep 3
/usr/local/sbin/faxmodem cua1

It's a Slackware 3.5 system (I'm watching cua1/ttyS1 developments)
