To use your own fax cover page, I recommend using the covers at
	ftp://ftp.sgi.com/sgi/fax/contrib/covers/
and modifying them as necessary.

Rather than viewing them directly with a Postscript viewer, I would
use

	faxcover -C template.ps -x "company name" -l "location" -r "subject" -c "comments" -t "recipient name" -n "fax number" >/tmp/faxcover.ps

Then use this result to see what your cover page *really* looks like.
